OCI APPLICATION SERVICES (Minors & Adults)

At Notary of Lewis Center, we provide comprehensive, end-to-end OCI application services for both minors and adults. We understand the complexities of the OCI process and ensure every step is handled smoothly with accuracy and professionalism.

Our services include:
• Professional Passport Photography — OCI-compliant, high-quality photos
• Document Scanning & Preparation — all required paperwork carefully reviewed and formatted
• Document Notarization — included in the OCI application fee

Required Documents for OCI Application

To avoid delays, please have the following original documents ready. Requirements may vary slightly depending on whether the applicant is a U.S.-born child or a child born in India who later naturalized.

For U.S.-born children applying for OCI (never held an Indian passport):

• Current valid U.S. Passport of the applicant

• U.S. Birth Certificate of the applicant (Must be apostilled from the state where applicant is born )

• Indian Passport of parent(s) or grandparent(s)

• Proof of valid U.S. visa status of Parents (e.g., Green Card, H-1B, L-1, etc.)

• Parents’ Marriage Certificate (must be apostilled in the state where issued if married in the USA)

• Proof of current U.S. address (driver’s license, utility bill, lease, etc.)

• If any family member already holds an OCI, their OCI details must be provided

• Relatives’ details in India (names, relationship, age, and current address)

• Visa to India if any

For children born in India and naturalized in the U.S. along with parents:

- Current valid U.S. Passport of the applicant

- Previous Indian Passport of the applicant

- Naturalization Certificate (if applicable)

- Indian Passport of parent(s)

- Parents’ Marriage Certificate

- Proof of current U.S. address

- If any family member already holds an OCI, their OCI details must be provided

- Relatives’ details in India (names, relationship, age, and current address)

OCI Application Preparation Fees:
• OCI Application Preparation Only — $100
• OCI Application + Passport Photo — $115

Additional Apostille Services (if required):

Ohio-born Children (Birth Certificate Apostille):
 • Standard — up to 5 business days — $50
 • Priority — 3–4 business days — $75
 • Expedited — same day or next business day* — $125

Children Born Outside of Ohio (Birth Certificate Apostille):
 • Standard Service Only — processing time is typically 10 business days or longer
 • Fee varies by state — please contact us for details
Parents Married in the USA (Marriage Certificate Apostille

- If marriage occurred in Ohio — same Apostille pricing as above (Standard, Priority, Expedited)

- If marriage occurred outside Ohio — Standard service only, with longer processing times (10+ business days); fee varies by state

Bundle Price Examples (Ohio Only):
